Employees of MCG can identify their veteran status and/or disability status.  This is strictly voluntary and will not affect employment status with MCG.

Employees of MCG and applicants for employment with MCG may identify their race, gender, and/or referral source.  Applicants must have already submitted an employment application prior to submitting this form.
